The Bay Community Volunteer Ambulance Corps, Inc. (“BCVAC”) loans orthopedic equipment free of charge to members of the community served by BCVAC. BCVAC does not provide instructions in the use of this equipment. Accordingly, persons intending to use such equipment should seek professional advice on the use of such equipment should seek professional advice on the use of the equipment prior to use. BCVAC shall not be liable for injuries resulting from improper or incorrect use of any orthopedic equipment loaned. While every effort is made to ensure that all equipment loaned is clean and in good working order, no assurance can be made by BCVAC that such equipment is free of defects that may prove harmful to the users of such equipment or to others. Commodes, Bedpans and other such items are not rented out nor will be excepted as a donation.

THE UNDERSIGNED HAS READ AND UNDERSTANDS THE ABOVE PARAGRAPH AND AGREES NOT TO HOLD BCVAC LIABLE FOR ANY USE OR MISUSE OF, OR ANY DEFECT IN, ANY EQUIPMENT BORROWED FROM BCVAC WHICH RESULTS IN INJURY TO THE USER OR ANY OTHER PERSON OR ANY PROPERTY. THE UNDERSIGNED FURTHER AGREES TO RETURN TO BCVAC ANY EQUIPMENT WHICH SUCH PERSON HAS OR IS CURRENTLY BORROWING FROM BCVAC IMMEDIATELY AFTER THE UNDERSIGNED NO LONGER NEEDS SUCH EQUIPMENT.
